Sorry, I’m still not sure what the issue you are having is. There is no form to fill out. Use whatever email server you prefer and simply send an email to support@eventideaudio.com. This will create a support ticket and you will receive emails back from our support team. You can then email them back. There is no password or account required for any of this.
Someone from our support team can help resolve your password issue, but you’ll need to follow the directions to email the support email address.
